About Us
The wellbeing outreach team (WOT) is non-profit vocational group set up to provide health and social care support to the local community in South London. It was set up by a group worshipers from the Old Kent Road Mosque who had a particular passion for using their experiences and expertise to serve their local community.
WOT is comprised of over 12 Executive members and over 20 associate members. Its members include Nurses, physiotherapist, Surgeons, Doctors (of multiple specialities), Midwives, Pharmacists, Social Workers, Occupational Therapists and much more.
The morals and ethics of islam underpin everything we do at WOT. The Prophet Muhammad (SAW), said “None of you will have faith until he loves for his brother or his neighbour what he loves for himself.” Good health, access to public services and support through difficult times are basic requirements that we’d all suffer without. Through WOT we are able to serve our community and do our best to ensure that nobody slips through the cracks
Services
Following consultation with our available healthcare professional, a referral could be made to an appropriate local healthcare facility such as GP Practices or depending on the severity Urgent care centre.
Housing support
We have social workers / care coordinators on hand to offer advice, support and referral to social services
Medication reviews
In-house pharmacists and Doctors are available to go through any medication queries
Post-discharge check-up
Our team offer calls and home visits to high-risk individuals after hospital discharge
Assistance with access to welfare support
We have social workers on hand to offer advice and support with applications to local councils
Befriending
Whether it's just a chat or a reminder to shut the windows, a member of the team is always on hand to be that friendly face
